My Nutrition Clinic Kidney Dietitian Dr Jaimon Kelly has a PhD in chronic kidney disease nutrition and has published over 78 publication and authored 2 book chapters on the topics of kidney and chronic disease management through nutrition. He is an experienced educator and is a Research Fellow at the University of Queensland and an Adjunct Assistant Professor at Bond University. Jaimon supervises dietetic student, for both clinic and research placements and delivers GP and nurse training, and public workshops.

Aloysa has over 30 years of experience as a Dietitian and has worked for Nutrition Australia and running her own private practice in Brisbane. In addition to her dietetic qualifications, Aloysa has a Graduate Diploma in Counselling from the Queensland University of Technology.

Having a counseling qualification alongside expertise in nutrition and food has led Aloysa to focus on the area of eating disorders and has a great deal of compassion and experience providing support for long-term recovery

A Chronic Disease Management Plan (formerly known as Enhanced Primary Care or EPC) is a Medicare program that provides rebates for allied health services, including dietitian consultations, for patients with chronic conditions.

Medicare provides rebates for up to 5 allied health visits per calendar year under a Chronic Disease Management Plan. These visits can be shared between different allied health providers.

Common qualifying conditions include:

  • Type 2 Diabetes
  • Heart Disease
  • High Blood Pressure
  • Weight concerns
  • Chronic Kidney Disease
  • Gastrointestinal Conditions (Note: Your GP will determine if you’re eligible based on your specific health conditions)

Schedule an appointment with your GP to discuss your eligibility. If appropriate, they will prepare a care plan and provide referrals to relevant allied health providers, including our dietitians.

This will depend on who you see as the fees will be reflective of experience and expertise. You can see all the fees and rebates here.

Yes, Medicare covers both face-to-face and telehealth consultations under a Chronic Disease Care Plan, making our services accessible across Australia.

Initial consultations typically take 45-60 minutes, while follow-up appointments are usually 30-45 minutes.

Please bring:

  • Your care plan referral from your GP
  • Any recent medical reports or test results
  • A list of current medications
  • A 3-day food diary (if possible)

The frequency of visits depends on your individual needs and goals. Your dietitian will discuss and recommend an appropriate schedule, typically ranging from weekly to quarterly visits.
